PIPC’s latest patient blog features Andrew Sperling from the National Alliance on Mental Illness. Mr Sperling provides his perspective on the importance of value frameworks in healthcare, and offers his concerns about ICER’s direction. “When organizations conducting value assessments initiate their work without engaging patients in the development of their fundamental research questions, we worry about its ultimate impact on affected patients. Without the perspective of patients at the table, the real-life benefits and risks of treatment that matter to patients cannot be identified and measured. Ultimately, if value is not measured from the patient’s perspective, patient access to important or necessary treatments could be impeded.”
“While we may worry that the design of ICER’s proposed review for [tardine dyskinesia] TD treatments will result in limited access to potentially promising therapies, we also know that organizations like PIPC, National Health Council, FasterCures, National Patient Advocate Foundation and others are working diligently on patient-centered value frameworks that are aligned with what matters to patients. 2. Chairman Coelho: Patient Engagement: The CHRONIC Care Act, CMMI, and the Future
PIPC Chairman Tony Coelho wrote a new blog for HuffPost, complimenting steps that are being taken that show progress in shifting toward value-based healthcare decisions. “While a tremendous amount of work remains to be done, policymakers took two steps recently that show progress is possible. One was approval by the Senate Finance Committee of the CHRONIC Care Act, a bipartisan bill (yes, they still exist!) whose success is attributable to significant stakeholder engagement and that, if enacted, will help to meet real patient needs.”
“We’ve also cheered regulators’ increasing responsiveness to the needs of patients. For example, the Department of Health and Human Services (HHS) recently signaled a small but important step in the right direction when they announced the creation of a patient ombudsman to help promote patient perspectives in Medicare’s shift towards alternative payment models (APMs). The new Alternative Payment Models Beneficiary Ombudsman will play an essential role in helping Medicare beneficiaries understand and navigate new “value-based” payment models developed through the Center for Medicare and Medicaid Innovation (CMMI).” Click here to read the blog.
3. PIPC Outlines Concerns with ISPOR's Value Assessment Frameworks Initiative.
Last week, Partnership to Improve Patient Care (PIPC) Chairman Tony Coelho submitted a letter to the International Society for Pharmacoeconomics and Outcomes Research (ISPOR) Chief Science Officer Richard Willke outlining “strong” concerns about the recent draft of a U.S. Value Assessment Frameworks initiative led by ISPOR. In the letter, Chairman Coelho expresses disappointment with ISPOR’s recommendations, noting that they “do not reflect advancement as indicated to be ISPOR’s mission.” “We understand the challenge of assessing value in health care in a manner that is centered on the characteristics, needs and preferences of the individual patient versus defining value based on what is cost effective for an ‘average’ patient, and appreciate that the work that the ISPOR Special Task Force on Value Assessment has undertaken is difficult,” wrote Chairman Coelho. “That said, we have significant concerns regarding the recommendations that ISPOR outlines in its draft report.” Click here to read the letter.
4. Healthcare Finance: Racial and ethnic disparities remain consistent at Veterans Health Administration patient-centered medical homes
Jeff Lagasse touches on the issue of racial inequity in patient-centered medical homes forHealthcare Finance. “The research, published in Health Affairs, began by examining data from 2009, which is when the Veterans Health Administration, a part of the Department of Veterans Affairs, began implementing patient-centered medical homes on a national scale -- what's known as the Patient Aligned Care Team Initiative. In that year, all groups -- African Americans, Native Americans, Hispanics, Asians and Pacific Islanders -- lagged behind caucasians when it came to care quality.” Click here to read the article.
5. AMJC Video: Dr L. Patrick James Discusses the Continued Move Toward Value-Based Payments
The American Journal of Managed Care released a new video featuring Dr. L Patrick James, who notes that the forces pushing the country toward value-based care will continue regardless of who is in control of Washington. “I attended some sessions yesterday at AHIP [Institute & Expo] and Dr Patrick Conway, chief medical officer of CMS, he avoided talking about what is going on politically right now, but he just reaffirmed that there's just so many things underway now in terms of the forces driving this. If Secretary Price has certain opinions about that, they certainly would get debated and aired in the political forum, but I think many people believe—I heard it over and over again yesterday, too, whether it was Joe Swedish of Anthem or Dr Eric Topol or former Governor Mike Leavitt—I think most experts in the area and all of us in the trenches, feel like these forces will continue. 6. PhRMA: Modernizing FDA Regulations to Advance a Value-Driven Health Care System
Michelle Drozd comments on the FDA’s push for more value-driven regulations. “...Allowing for broader communication from biopharmaceutical companies to payers and health care providers about the most current, accurate and comprehensive information available on FDA-approved and investigational medicines can help these key stakeholders make more informed decisions for the benefit of patients. It can also support the move toward value-based contracts for pharmaceuticals by allowing the conversations needed to get to those contracts.” Click here to read the article.
7. Modern Healthcare: House Lawmakers Ask How Medicare Can Manage Complex Patients Better
Mara Lee notes that lawmakers probed on how Medicare can manage complex patients better in a Ways and Means subcommittee hearing last week in Modern Healthcare. “...The other approach discussed was allowing Medicare Advantage plans to charge different co-pays to different groups of patients for certain services, or use co-pays to steer patients to higher-quality providers. Dr. Mark Fendrick, director of University of Michigan's Center for Value-Based Insurance Design, said Congress should give insurers that authority beyond a pilot program. He also suggested lawmakers give insurers the option to reduce or eliminate cost-sharing for people with chronic diseases who are cooperating in a disease management program.” Click here to read the article.
8. Hepatitis C Mentor and Support Group Releases Psychological Survey Results
The Hepatitis C Mentor and Support Group released its findings from its recent Psychological Survey. “Aside from the physical effects surrounding Hepatitis C, the psychological effects on patients, family, and friends have been overlooked. The Hepatitis C Mentor and Support Group recently conducted a survey focusing on just that. These emotional effects include but are not limited to depression, sadness, denial, worthlessness, anger, stress, a feeling of helplessness, guilt, anxiety and so many more. During this survey HCMSG received hundreds of compelling testimony from those affected, directly and indirectly. HCV does not discriminate.” Click here to read the report.

The American Association on Health and Disability’s Frederick J. Krause Scholarship on Health and Disability is currently accepting applications. The scholarship honors Frederick J. Krause, a lifelong advocate for those with disabilities who passed away in 2014. Beginning with his role as a special education teacher, Mr. Krause’s distinguished career in advocacy included service in the federal government and the cofounding of the American Association on Health and Disability.
